ID Requirements

PLEASE READ CAREFULLY

If over 18…

-We require a valid (non expired) government issued photo ID for ANY service
             -Acceptable forms of government issued photo ID include
                 -Driver’s License/Permit
                 -Passport
                 -Military ID

If under 18…

-We require a valid (nonexpired) government issued photo ID from BOTH parent and client
              -Acceptable forms of government issued photo ID include
                    -Driver’s license/permit
                    -Passport
                    -Military ID

Last names and addresses must match on the provided government issued photo IDs from both parent and client. If they do NOT match you will need to bring the following in ADDITION to the government issued photo IDs…

         -original birth certificate of client (we do NOT accept copies or photos)
           -Marriage certificate (if showing a name change, we do NOT accept copies or photos)

If the client does NOT have a valid government issued photo ID you will need to bring in…
              -Original birth certificate of client (we do NOT accept copies or photos)
              -Photo ID for client (recent photo with name printed- school/yearbook photo, parentVUE)
              -Parent's valid government issued photo ID


*If at the time of the appointment you do not have the correct IDs your deposit will not be refunded
